Efficacy of ACTH Treatment in Infantile Spasm: a Pilot Study for Differential Effects between the High-dose versus Low-dose Therapy


Abstract
Background:
@EN Although it is well known that ACTH is effective in the cessation of spasm in the patients with infantile spasm, there are disagreements and controversies concerning the dosage and duration of the treatment. This study was performed to
analyze
the
differential effects in the response and long term course between the high-dose versus low-dose ACTH therapy.
@ES Method:
@EN The medical records of the 73 patients with infantile spasm who met the inclusion and exclusion criteria were reviewed. The responses, relapses, side-effects and long-term courses between the high-dose and low-dose group were compared.
@ES Results:
@EN Of the 41 symptomatic and of the 32 cryptogenic patients. 28(68.3%) and 27(84.4%) responded respectively. Of the 43 patients treated with high-dose therapy, 33 (76.6%) responded: of the 30 patients treated with low-dose therapy, 22(73.3%)
responded. The side-effects, especially hypertension and infection, occurred more frequently in the high-dose group.
@ES Conclusion:
@EN We concluded that there is no significant difference in the response rate between the high-dose versus low-dose group in the treatment of infantile spasm. But care should be taken for the occurrence of the side-effects when treating infantile
spasm
patient with high-dose ACTH regimen.
